Welcome New Wolves!

Please join us for our Registration Celebration from 6:30 p.m. – 8:00 p.m. To help minimize wait time, we recommend families come to the library to finalize registration during the following times (by last name):
 6:30 p.m. – 7:00 p.m.: A – G
 7:00 p.m. – 7:30 p.m.: H – O
 7:30 p.m. – 8:00 p.m.: P – Z

Please bring the following the night of the Celebration:
 Original Birth Certificate (We will make a copy for our records)
 Copy of Catholic Baptismal Certificate (if applicable)
 Completed Freshman Elective Preference worksheet.

Please complete & submit electronically the following Checklist items before the Registration
Celebration:
 Enrollment Contract
 Creation of a FACTS account for JP (if you have not yet established one)
 Paid $500 non-refundable deposit
 Office of Catholic Schools/Diocese/Confidential Student Health History
 Commonwealth of Virginia School Entrance Health Form (print & submit to your doctor)
 Diocese of Arlington Permission for Emergency Care Form
 High School Over the Counter (OTC) Medication Authorization
 Photo, Press, Audio, and Electronic Release
 Immunization History
 House Information Form

The Diocese of Arlington enrollment deadline is Saturday, March 17, 2018. Late registrations will be accepted, pending available space. Seats and financial aid awards are not guaranteed after the deadline.

If you cannot make it to the Celebration on March 15th, please submit the Checklist items listed above electronically and make arrangements to bring the original birth certificate and the copy of the Catholic Baptismal Certificate (if applicable) to school before the deadline.
